I {name} (hereinafter “Releaser”) voluntarily submit myself for attendance and participation in martial arts classes and fitness classes and instruction by Brazos Valley Mixed Martial Arts and it's designated agents and affiliates (hereinafter “BVMMA”). Releaser acknowledges that physical fitness training and martial arts are inherently physical in nature and that injuries to Releaser and/or their children can and will occur at some point, even with the utmost care and caution exercised by BVMMA. Releaser assumes full responsibility for any and all damages, injuries or losses that I, or my children, may sustain or incur, if any, including complications from Covid-19 or other viruses while attending and/or participating in these sessions and classes--including any time between classes in or around the BVMMA training facility, and at any events hosted by, attended by, or in relation to BVMMA. This Release Form between BVMMA and its members, and party of the below signature is an agreement between all parties. Releaser agrees to fully and forever waive and release BVMMA from any and all claims for damages, injuries and/or losses that occur in relation to any and all activities associated with BVMMA. Releaser is responsible for attending an introductory course or private lessons prior to engaging in any sparring activities. Releaser fully understands that any medical treatment given to me will be First-Aid treatment only and formally requests that BVMMA administer whatever First-Aid they deem necessary. Releaser further states that he/she has carefully read the above Release, knows the content of the Release, and chooses to sign this release on his/her own free act. If under 18, I understand that I cannot participate in any activities within or related to Brazos Valley Mixed Martial Arts until this release and consent is signed by my parent or legal guardian.

Additionally, I consent that any pictures or video taken of me by BVMMA or in connection with classes or practice can be used for any purpose, including, but not limited to: publicity, promotion, marketing, advertisement etc. by BVMMA and I waive any and all compensation in regard thereto. I release BVMMA from any claims or causes of action that may arise such use of my image, including, but not limited to: rights of publicity, copyright, invasion of privacy, defamation, etc. I hereby acknowledge that I have read, consent, and agree to abide by all the waivers and stipulations above.
